The Super Student Awards Event Nomination Process

Those interested in submitting a nomination for the Super Student Award should follow the process noted below.  All information/items must be submitted and required signatures must accompany entry.  Mail or email items to Mrs. Kim Scott c/o the contact info provided at the left hand side of this page.  Thank you for your interest!

Males and females (ages 10-24 years old) are eligible to participate in the Super Student Awards Event.  No cost to nominees or nominators.


ATTENTION NOMINATORS:
Please be sure to include the following in your nomination:

• 1-Name
  2-Business Address
  3-Cell and/or business phone
  4-Email address
  Your association with nominee: school staff, church leader, employer, other (specify)
  How many years have you known nominee?
  Nominee info needed:
       Provide same as above # 1-2-3-4, including nominee address
       Age, DOB, school, grade level, graduation year, GPA
       Volunteer work: company name, dates
  
  Parent permission (signature needed) to participate in the Super Student Awards Event: Signature Required, Include Date Signed.
  A photo of nominee is required.
  -1-page summary---Explain why this student deserves a Super Student Award (remember the judging categories)
  -1 letter of reference (other than the nominator) - to add to the application
  
  *Nominator must submit business card (contact info).

IMPORTANT - VIEW THE TEEN FOCUS SUPER STUDENT AWARDS INFORMATION BELOW
No cost to nominees or nominators
***For males & females ages 10-24***
-This is an annual event that recognizes and rewards students for good choices and positive behavior
-Students are nominated by (only one nomination is needed) school staff, church leader, employer, anyone that can evaluate nominee's character
-All nominators will need to provide Mrs. Scott with specific contact information.

Below are the subjects & judging categories. Nominators must complete an online application and choose a category (based on your nominee's story) such as:
---Student Success-grades/attendance/ discipline/student activities  
---Character Education-respect for self & others/kindness/selflessness/sacrifice/
attitude/follows rules & regulations, etc.
---Career Awareness-has a solid plan for their future/understands what it takes to achieve career goals/on a path to reach career goals
---Teen Focus Volunteer Award- Mrs. Scott & Teen Focus representatives will nominate these individuals.
 
The Teen Focus Super Student Awards Event is similar in style and format to the television awards shows.
